As required by the Environmental Protection Agency (EPA), we are conducting this Lead Service Line Inventory for both the water system-owned and customer-owned portions of the water service lines. If you have the capability to take a photo of your water line where it enters the house and attach it to the PDF fillable file, that would be appreciated as well!
